Custom Response Types in monitorQA allow you to design your own answer sets that can be reused across multiple templates. Instead of using only the standard Yes/No or Pass/Fail options, you can create responses that better match your organization’s processes and scoring needs.
Steps to Create a Custom Response Type

Navigate to Custom Response Types
- Log into the web app.

From the left-hand menu, go to Library and select Custom Response Types.

Create a New Response Type

On the Custom Response Types page, click Create Response at the top right.

Name Your Response Type
- Enter a name for your response type

Note: This name will not appear in the audit question itself, only in the list of available response types.

Choose Format
- Select whether the responses will display as Buttons or a Dropdown in the audit.

Add Answer Options
- You can add up to 6 possible answers. For each answer, assign:
- A score value.
- Whether it is a failed response or not.

Save Your Response Type

Once all options are set, click Create to finalize.

Applying Your Custom Response Type
- Open the audit template you want to edit.
- For any item, under Response Type, select the custom response type you just created.
- (Optional) Update the score values directly in the template if needed.

Publish the template.
Viewing During an Audit
Once published, your custom response type will appear in the audit exactly as configured (buttons or dropdown), and responses will apply the scoring and fail logic you set.
